Weight-bearing radiographs are essential, but severe-looking osteoarthritis can be relatively well tolerated. The operation is not scheduled by an imaging grade alone.
Why MRI can mislead
Degenerative meniscal and cartilage findings are common in middle age. They may be real without being the main source of pain.
Non-surgical care
Strengthening, physical activity, weight management when relevant, medication and selected injections can make function acceptable for long periods.
Choosing surgery
Osteotomy, partial replacement and total replacement answer different mechanical problems; alignment, ligaments and compartment involvement matter.
